As of July 1, 2024, the New Jersey Department of Education revised the guidelines for sports physicals as well as the following Student Athlete Preparticipation Forms:
Preparticipation Health History Form
Athletes with Disabilities Form
Physical Examination Form
Medical Eligibility Form
These updated forms, LINKED HERE, are required for any student athlete whose annual physical exam is completed after June 30, 2024.
To ensure a smooth start for all of the student athletes, we wanted to prepare you and your child with this very important information for your planning. It outlines the new forms with links, important due dates, general guidelines and information for Metuchen High School athletics for the 2024-2025 school year.
